We want your visits to Desert Palms Physical Therapy to be pleasant and productive. Let us give you an
idea of what to expect on your visits.
For your first visit, plan to arrive 15 minutes before your scheduled appointment time. Please download and complete the 5 forms you see listed below. These forms will provide your Therapist with specifics about your medical history, injuries, pain, and current function.
- Bring your Doctor Order or referral.
- Bring your insurance card or complete insurance information.
- Wear loose fitting clothes.
APPOINTMENTS
Desert Palms Physical Therapy is open Monday thru Friday from 8:00AM to 6:00PM.
We have evening hours available by appointment. We will do our best to accomodate you needs.

YOUR INSURANCE
Medical insurance generally covers physical therapy; however, because each insurance plan is
different, you should check with your insurance company to determine your benefits. You
may contact our staff who will be glad to assist you in determining your level of coverage and any
out of pocket expense you will have.
As a courtesy, we will submit insurance claims on your behalf. It is your responsibility to provide us with accurate insurance coverage information to ensure your insurance company properly processes your claims.
PAYMENT
We will collect your copayment, co-insurance or deductible at the time of treatment. We accept payment via Cash, Check, Visa, Mastercard or Discover card.

Each Therapist shares a commitment

PHYSICIAN REFERRALS
A Doctor order for treatment may be written by a Medical Doctor (MD), Osteopath (DO), Podiatrist, Chiropractor, Physician Assistant or Nurse Practitioner.
SELF REFERRALS
In Arizona, you may be treated by a licensed Physical Therapist without Doctor's orders. However, be aware most insurance companies will not pay for these treatments without written Doctor's orders or referral.
LIENS FOR AUTO ACCIDENT OR PERSONAL INJURY
If you have been injured in an auto accident or have a personal injury claim, there are options for coverage of your physical therapy treatment:
Attorney Lien
For qualified Attorney /Patient liens for auto accidents or personal injury claims, we will need the name, address and telephone number for your Attorney. You will be asked to sign a lien document we prepare which is also signed by your Attorney. This lien is a contract which states your Attorney will reimburse Desert Palms PT when there is a settlement in the case. If there is no settlement, you will be responsible for paying the charges on your account.
3rd Party Insurance
The person who caused your accident will have insurance that could cover a portion of the cost of your physical therapy treatments. You will be asked to sign a medical lien document, this document is filed with the Pima County court system. The lien is a contract which states the 3rd party insurance will reimburse Desert Palms PT for the treatments provided. If there is no settlement, you will be responsible for paying the charges on your account.
DISCOUNTS
We provide the option of arranging a discounted payment plan to Cash pay patients.
